Permission & Consent
Trip Participation Consent
give permission for my child to participate in the school ski trip, including transportation and participation in skiing/snowboarding activities.
Medical Treatment Consent
I authorize school staff to obtain medical treatment in the event of an emergency.
Liability Waiver Agreement
I agree to the privacy policy.
1. Assumption of Risk
I understand that skiing and snowboarding are physical activities that involve inherent risks, including but not limited to slips, falls, collisions with other participants, equipment failure, weather conditions, and other hazards that may result in injury. I acknowledge that these risks cannot be completely eliminated, even with proper supervision and safety measures in place.
2. Voluntary Participation
I confirm that my child is voluntarily participating in the school ski trip and that I believe they are physically able to take part in skiing or snowboarding activities.
3. Responsibility for Behaviour
I understand that my child is expected to follow all school rules, trip guidelines, staff instructions, and resort safety rules. I acknowledge that failure to follow these rules may result in my child being removed from activities, with transportation arrangements made at my expense if necessary.
4. Medical Consent
In the event of an accident or medical emergency, I authorize school staff to seek medical treatment for my child, including first aid, ambulance transport, or hospital care if required. I understand that I am responsible for any associated medical costs.
5. Release of Liability
To the fullest extent permitted by law, I hereby release the school, its staff, volunteers, and trip supervisors from liability for any injury, accident, loss, or damage that may occur during the ski trip, except in cases of gross negligence or intentional misconduct.
6. Equipment Responsibility
If my child is bringing personal ski or snowboard equipment, I understand that the school is not responsible for loss, theft, or damage to the equipment.
⸻
By checking the consent box below, I confirm that I have read, understood, and agree to this Liability Waiver Agreement.
Code of Conduct
My child agrees to follow all trip rules and behavior expectations.
1. Behaviour Expectations
Students must behave respectfully and responsibly at all times. This includes following instructions from teachers, trip supervisors, and ski resort staff. Polite behaviour, cooperation, and positive attitudes are expected throughout the trip.
2. Safety Rules
Students must follow all safety rules provided by the school and the ski resort, including:
• Staying within designated areas
• Wearing helmets when required
• Skiing/snowboarding within their ability level
• Reporting any injury or incident to staff immediately
Reckless, dangerous, or irresponsible behaviour will not be tolerated.
3. Group Supervision
Students must remain with their assigned group or meeting points and must check in at designated times. Leaving the group or going off on their own is not permitted.
4. Respect for Property & Equipment
Students must treat all rented and personal equipment with care. Any intentional damage or loss may result in parents being responsible for repair or replacement costs.
5. Personal Conduct
Students must use respectful language and maintain appropriate behaviour at all times. Bullying, harassment, or inappropriate behaviour of any kind will result in consequences and possible removal from the activity.
6. Rules on the Bus (if applicable)
Students must follow all bus safety rules, including staying seated, listening to the driver, and keeping aisles clear. Disruptive behaviour is not permitted.
7. Consequences for Misconduct
Students who fail to follow the Code of Conduct may:
• Be removed from ski/snowboard activities for the day
• Have parents contacted to arrange early pickup
• Face school-based disciplinary action upon return
⸻
By checking the consent box below, I confirm that I have reviewed this Code of Conduct with my child, and we both agree to follow all expectations during the ski trip.
